Bleeding

Bleeding hemorrhoids in Montreal are assessed

Bleeding is the commonest reason people come here, and it is also the symptom this clinic is slowest to draw conclusions about. The examination comes before the reassurance, not the other way round.

  • Never assumed to be benign
  • Family history asked every time
  • Referred on when it is not us

Surface or mixed

Where the blood sits tells us something

Bright red blood sitting on the surface of the stool, on the paper or in the bowl is the pattern hemorrhoids usually produce. Blood mixed evenly through the stool suggests a source higher up, and that moves the question away from this clinic entirely.

It is a useful distinction and a long way from being a conclusive one. Darker blood, black or tarry stools, and bleeding alongside a change in stool shape or frequency all point elsewhere too, and none of that is something you can reliably sort out by looking at it yourself.

So the first visit assesses rather than reassures. The red flags page lists what gets a referral instead of a procedure, the examination page describes what is actually being looked for, and fissures cover the other common cause of visible blood.

The route

How bleeding is worked through in Montreal

  1. The pattern

On the surface or mixed through, how often, how much, and whether anything about your stool has changed.

  2. The history

Age, family history of colorectal cancer, weight loss, medications and blood thinners. Any of these changes what happens next.

  3. The examination

An external look and an anoscopy, which is what separates hemorrhoids from a fissure and both from something else.

  4. Treat or refer

A procedure with a plan behind it, or a referral for a colonoscopy, decided at that visit rather than later.

Not drawing conclusions

Why we are slow to reassure

Rectal bleeding is the leading symptom of hemorrhoids. It is also a presenting symptom of colorectal cancer. Both of those are true at once, which is the entire reason this page is cautious where a sales page would be comforting.

The failure mode here is not treating hemorrhoids that turn out to be hemorrhoids. It is someone being told early and confidently that there is no cause for concern, going away for a year on the strength of it, and arriving back with something that was treatable twelve months ago.

That is also why months of cream on bleeding nobody has examined is itself a warning sign, whoever suggested it. The creams guide says so plainly, internal disease explains why painless bleeding is so easy to leave, and referrals cover what happens when we are the ones making one.

Common questions

Bleeding hemorrhoid questions in Montreal

The questions worth asking before anyone decides this is nothing, including the uncomfortable ones.

Commonly, and commonly is not always. Bright red blood on the surface does point that way, which is a reason to have it examined and confirmed rather than a reason to leave it.
Heavy or continuous bleeding, clots, dizziness, fainting or a racing pulse need urgent care now. Short of that, the amount matters less than how long it has gone on and whether it keeps returning.
Bleeding that stops and then comes back is one of the patterns that should be looked at, not one that should be filed as resolved. Recurrence is information rather than reassurance.
Yes. Being over forty-five, or having a family history of colorectal cancer, changes what has to happen before any procedure. It reorders the plan rather than ruling treatment out.
Sometimes, and we will say so at the first visit if the findings or your history point that way. We also expect an endoscopic test within a reasonable interval before banding.
It does, both for how the bleeding is interpreted and for what can safely be done about it. Mention it early, because it changes the screening and sometimes the choice of procedure.

Bleeding

Why this one gets examined first

Two true things at once

It is the commonest hemorrhoid symptom and a presenting symptom of colorectal cancer. Both facts hold.

Position is a clue

On the surface points one way, mixed through the stool another. Useful, and nowhere near conclusive.

History asked every time

Age, family history, weight loss and stool changes are asked at every first visit, not just when something seems off.

Cream for months is a flag

Treating unexamined bleeding for months is a warning sign in itself, whoever suggested the cream.
